It's the last chance for the Manx public to have its say on the future of end of life care on the Island.
Over 800 replies have been submitted so far, with the consultation closing on Monday.
The feedback is to be analysed over the next few months, with a strategy published by the Department of Health next year.
The questionnaire can be picked up from GPs, the Sea Terminal, libraries, Commissioners' Offices and pharmacies
